Our patients and their families are the reason for everything we do. That means providing them with the utmost physical, emotional and spiritual care — through every step of the hospice journey.
What is Hospice?
Hospice care focuses on the quality of life for patients who are experiencing advanced, life-limiting illness or disease. Care focuses on managing symptoms, providing social and emotional support, as well as guiding families through this difficult process.
The Quality Care team of professionals works to offer comfort and companionship for the patient, plus support and respite for loved ones. This special phase of care aims to enhance and dignify the final stages of life by providing personalized help and expert resources for the end-of-life journey.
Your Quality Care Team
Every one of our patients is assigned a care team consisting of each of the following disciplines:
- Physician
- Registered nurse
- Social worker
- Hospice aide
- Chaplain
- Bereavement specialist
- Volunteer
Additional team members may include massage therapists, as well as physical, occupational and/or speech therapists, depending on need.
Our Services
The experienced Quality Care team will make sure both patients and their families have everything they need during the hospice journey. Care services include, but are not limited to, the following:
Management of symptoms such as pain, nausea, vomiting, shortness of breath and restlessness
Care coordination with other providers involved in your care, including your primary care physician
Medication management for complete and accurate care
Personal care such as help with bathing, eating, and caring for your skin, hair and mouth
Spiritual care using the insights and principles of psychology, religion, spirituality and theology
Companionship for the patient
Grief counseling for patients, families and other loved ones
